Once you're registered for a summer camp, please carefully review the information below and complete the necessary camp forms.
1. SUMMER CAMP PERMISSION & RELEASE - required for all students attending half and full day programs
2. AUTHORIZATION FOR ADMINISTRATION OF MEDICATION - required for all students requiring medication during camp hours (including all students who carry an epi-pen)
3. PIZZA ORDER FORM - full day programs only

5. WEEKLY CALENDARS - Find the calendar for the week that your child is attending below.
